Complete the Registration Form

Site Visitor applicant completes the required fields of the profile.

Required Fields:

     Personal Information

·         First Name

·         Last Name

·         Email

·         Confirm Email

·         Password (must be at least six characters long)

·         Confirm Password (must be at least six characters long)

     Note:  be sure to save your password

     Contact Details

•         Address Line 1

•          City

•         State/Province

•         Country

•          Zip/Postal Code

•          Office Phone

Optional Fields:

·         Middle Initial

·         Address Line 2

·         Address Line

·         All phone numbers

Once all required field are completed, click “Register.”  This will open the sections to be completed for the Site Visitor application

You will receive an email from APA CoA Portal letting you know that the registration has been received with your user name and password.  It will tell you what URL you should proceed to for completion of the Application.
